  1. Read Referrer's Information Pack

  2. Contact Upside team

  3. Referrer's agreement signed

  4. Referral form submitted

  5. Young person criteria approved

  6. Young person and whānau/caregiver interview

  7. Young person approval

  8. First meeting with mentor
